Heart-Full Minds Counseling LLC is invested in your mental health needs and relationship-based concerns. If you choose Roxanne Nowak as your therapist, you will receive:

  • Personalized and compassionate care

  • Evidence-based treatment approaches customized to fit your needs

  • Individualized treatment plans and goals

  • A direct approach to help you solve or manage your concerns

  • Timely treatment reviews/evaluations (as needed/changes occur) to help keep you on track of your goals

  • A “Good Faith Estimate” explaining how much your medical care will cost (Please see below).

Payment Options:

  • I accept all major credit/debit cards, as well as HSA/FSA cards (if determined to be eligible by your provider - see above).

Cancellation Policy:

  • Since scheduling a session involves reserving a time specifically for you, a minimum of a 24-hours notice is required to reschedule or cancel an appointment.

Good Faith Estimate

Under the No Suprises Act (which went into effect on January 1, 2022), health care providers need to give clients or patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes (under the law/when applicable as this private practice only participates in an outpatient level of counseling services) related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. Heart-Full Minds Counseling LLC will provide an estimate in the first pages of your intake paperwork which will all need to be completed at least 24 hrs. prior to your first appointment. You can also request a Good Faith Estimate before the intake process if you prefer.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
